The product includes an easy to use reporting form to capture and report security relevant information.  Individuals who are a witness to or involved in a security incident should complete the submitter portion of the form.  The form should be forwarded to a designated person or organization (e.g., security officer, site manager, HR, CIRT, etc.) for appropriate action and/or archiving.
